Team representative or Manager will need the following for check-in:

A copy of their roster signed by the registrar and coach.

Notarized medical releases for every player.

A USYSA coach’s ID card (provide by your home association)

6 copies of the misconduct game report. Must be TYPED. (XLS or PDF)
