Coaches will need the following for check-in:
- A copy of their roster signed by the registrar and coach
- Team information sheet (XLS)
- Notarized medical release form for every player including guest players
- The copies of the guest player release forms previously sent in to NTSSA (doc or pdf)
- A USYSA coach’s ID card (provide by your home association)
- 7 copies of the misconduct game report. Must be typed.
