A coach or team representative will need to check in 1 hour prior to the start of your first game at your assigned complex.
The following items are needed for check-in:
• A copy of their roster signed by the registrar and coach
• Completed Team Check-in Sheet XLS or PDF
• Notarized medical releases for every player including guest players
• Guest Player Release Forms - DOC or PDF
• A USYSA coach’s ID card (provide by your home association)
