Team representative or Manager will need the following for check-in:
- A copy of their roster signed by the registrar and coach
- Team information sheet. DOC or PDF
- Notarized medical releases for every player including guest players
- Guest Player Release Forms - DOC or PDF
- 7 copies of the misconduct game report. Must be TYPED. (XLS or PDF)
- A USYSA coach’s ID card (provide by your home association)
